· Our consultation fee is $69. This fee is required to schedule a consultation with our provider.

· Payment is due before or after consultation appointment

· The cost of additional services will vary depending on the type of treatment recommended during the consultation and the individual needs of each patient.

3. Payment & Payment Plans

· Payment for the consultation fee is due at the time of booking the appointment.

· Payment for treatments and services will be discussed during your consultation and is due at the time of service unless otherwise agreed upon.

· We accept payment via credit card, cash or check.

· If you choose to do a 6-month payment plan, we will have you sign a contract that you agree to the terms and paying the down payment and over the 6-month period. Payment plans will depend on type of treatment and how many sessions you want, so not all payment plans are the same.

· Should you choose a 6-month payment plan, your down payment is due at the time of your appointment. We will discuss with you the autopayments will occur (date of the month) and the amount.

4. Cancellations and Rescheduling

· If you need to cancel or reschedule your consultation or appointment, please notify us at least 24 hours in advance.

· Failure to cancel or reschedule within the required time may result in the forfeiture of your consultation fee.

· If you miss an appointment without prior notice, we reserve the right to charge a cancellation fee or refuse to reschedule.

5. Patient Responsibilities

· You are responsible for providing accurate and complete information about your medical history and current health conditions during the consultation and treatment process.

· You must follow all post-treatment care instructions provided by our provider to ensure the best possible outcomes.
